

Always get the information you need with required fields. Patients can only submit their information to you when they complete all of the required fields. Never receive an incomplete new patient form again. When a patient attempts to submit an incomplete form, they will be instructed to go back and complete the missing information.

Not all fields may be applicable to the patient, so designating required fields will ensure your patient is giving you what is needed, every time.


If patients need to sign in multiple places, it can be time consuming to have the patient go back through the form to collect any missing signatures.
